Out-of-network: A superbill can be provided for you to give to your insurer. Please check with your insurance company regarding out of network benefits and reimbursement.
GENESIGHT
®
Your insurance will be billed for the cost of the GeneSight test. You may be expected to cover a portion of the cost (typically $330 or less).
CANCELLATION POLICY
Please provide 24-hour notice for cancellations. This policy helps patients access the care they need when they need it. If you are unable to provide the 24-hour notice, the full fee for your appointment will be charged to the form of payment on file.
